Summary

The Design and Implementation scorecard template is a basic Design for Six Sigma (DFSS) tool that enables a Lean Six Sigma Black Belt to predict the future Design Performance of a product or process at implementation time.

To achieve that, the template provides both a Predictive Design scorecard and a Verified Implementation scorecard which allows a product/process design made of up to ten Continuous CTQs and ten Discrete CTQs.

Scorecard includes Long Term defect rate, Long Term Z value, Short Term defect rate, Short Term Z value. Also included is a detailed instruction sheet to help you get familiar and start using the scorecard right away. In addition to the instructions, there is a completed template you can use as a sample illustration.
